POSITION OVERVIEW
School Based Clinician
The School Based Clinician is responsible for providing clinical services to youth and their families according to program and regulatory guidelines, ensuring that all services are strengths-based, individualized, and comprehensive according to need.
RESPONSIBILITIES:
- Provide clinical services to youth and their families as assigned.
- Participate in the integrated service planning process for all youths enrolled.
- Ensure all service plans are strengths-based, individualized, and comprehensive according to need.
- Monitor the effectiveness of integrated service plans and review.
- Ensure all service are coordinated with other service providers.
- Access supports in the School or in the community as indicated.
- Assist families as needed in accessing assessments and recommendations for psychotropic medication.
EDUCATION and/or EXPERIENCE REQUIREMENTS:
- Requires a Masters degree, MSW or equivalent.
- Experience, training, or education commensurate with working with children with serious and persistent emotional disturbance is desired.
- Experience with working in a school environment is preferred.
- Must be licensed prior to starting, or have an active limited permit. LMSW, LCSW, or LCSW-R preferred.
- Required to pass licensing exam within six months of employment if working with a limited permit.
- Drivers license is required.

Job Summary | Endowed Constellation Faculty in Future Chips at Rensselaer Polytechnic Institute We are especially interested in candidates who have a demonstrated ability to perform high impact research in semiconductor technology to address the next generation challenges in computing. Areas of focus are rooted in fundamental science and engineering that drive advances in microelectronics, such as heterogeneous integration, advanced logic, advanced packaging, and power electronics. Physics of materials is also important to making these advances and is, therefore, an area of interest. The ideal candidates are expected to develop an internationally recognized, externally funded, scientific program in chip technology for the future of computing. As a Community Service Provider, you will provide home and community-based services to help children with emotional and mental health challenges. You will travel locally to work with children and families in their communities providing a core of rehabilitative services to ensure success, including non-licensed counseling and support for youth and family, skill building, peer support, caregiver support and advocacy, pre-vocational services and respite. Dietician – Per Diem
Northern Rivers has an opening for a Per Diem Dietician averaging ten hours a month to assist in guiding our food and nutrition program for residential youth clients.As a Dietician, you will be responsible for the consultation and review of food and nutritional services to provide nutrition therapy including nutrition assessment, diet modification, nutrition education, and intervention for the children residing in our residential housing. You will report to the Director of Food Services and you will work remotely.
Responsibilities:
- Encourage healthy eating habits for sustainability outside the agency
- Complete 11-day initial assessments per OMH regulations
- Develop menus for special diets (e.g.: gluten free, diabetic) agency wide.
- Work following the standards set for New York State (NYS) Departments of Mental Hygiene, Office of Children and Family Services, Office of Mental Health, and the United States Department of Agriculture.
Requirements and Education:

Job Summary | The Sr. Major Gift Officer (MGO) will serve as the Senior Regional Director of Development for the Southwest Region of the United States and will be responsible for developing a regular cadence of strategic opportunities focused on engaging existing and prospective major donors with the end goal of qualifying, soliciting, and closing major gifts. Clinician, Youth ACT
Licensed Social Workers are eligible for a $7500 signing bonus
As a Clinician in the Youth ACT program, you will help children avoid hospitalizations and out-of-home placements by providing intensive, community-based services to these youth and families. The youth served by Youth ACT have not found success in traditional mental health and community treatment. Our Clinicians are part of a tight-knit multi-disciplinary team that meets daily to provide medication treatment, therapy, skill building, and psychoeducational support to youth and families to meet all of their needs. You will report to the Director of Youth ACT and you will work on-site and in the community traveling to client's homes required.
Responsibilities:
- A shared caseload and ample supervision and support to provide intensive services to the 36 youth and families that the team serves overall, meeting the client where they are in the community, rather than in a clinical office setting.
- Specialized training in the Youth ACT model as well as the principles of person-centered planning, motivational interviewing, trauma-informed care, social determinants of health, cultural competence, implicit bias, and many other best practices.
- A lead role in providing services to clients/families such as:
- Risk Assessment
- Substance Abuse Screen/Assessment
- Suicide Assessment
- Mental Health needs assessment
- Social Needs Screening - Health Leads
- Comprehensive Psychosocial Assessment to include diagnosis and needs to develop a treatment plan (IAP)

Description
Behavior and Campus Support Worker
You will ensure the security of campus and community residential locations, including working with staff, maintaining communication with resident’s family and guardians, and overseeing resident transportation needs. You will report to the Online/Oncall supervisor and you will work on-site.
Responsibilities:
- Oversee and ensure implementation of all applicable emergency systems, agency policies, and program procedures.
- Respond to crises within assigned Residential Program/locations. Ensure response follows agency policy and protocol appropriate to the situation, and ensure that the individual clients behavior management plan(s) are followed. Ensure any physical intervention is conducted following TCI standards. Ensure completion of all forms and appropriate documentation of every restraint.
- Provide support to teams when asked for behavioral interventions and events. Will direct the team in absence of residential supervisor and will be a source of training , supporting and modeling for other staff.
- Ensure the security of campus and community locations, responding to any security concerns. Conduct random checks of assigned locations to ensure safety of residents.
- Be the primary person in charge of calling the police for any resident considered missing, completing or ensuring completion of the Missing Person Report form and canceling Missing Person Reports upon a residents return.
- Ensure contacts have been made with the residents family and guardian for all significant incidents.
- Maintain regular entries in the Online/On call log regarding all significant events that occur, and provide the leadership with specific additional information as. Ensure completion of documentation for all significant incidents.
- Ensure significant incidents are called into the Justice Center as applicable.
- Serve as the central person in charge of off campus trips. Approve and log all off-campus trips and assure such trips have proper staff coverage.
- Oversee and transport clients for medical, mental health, and other essential needs.
- Check and assure that all visitors have prior approval to be on the grounds.
- Be available to hear and document any neighbor concerns.
Requirements and Education:
- Requires an Associates degree and Bachelors degree and equivalent experience preferred.
- 3 years of experience, or equivalent education and experience.
- Drivers license is required.
- Must be able to pass the required physical.
- Must be able to complete and pass TCI training.

Per Diem Clinician, Permanency Resource Center (PRC)
As a Per Diem Clinician, you will provide trauma-informed family therapy, child therapy, group therapy, and crisis intervention as indicated to individuals, families and groups in a Permanency Resource Center. This can include outpatient individual or family therapy, group therapy, consultation to caregivers, and support to case managers. Work is designed to be short-term but will follow the needs of the family as written in their service plan. You will be assigned between 2 and 15 cases. Therapy is to be provided in the community or in office, and is paid for by the program, not through insurance reimbursement. You will report to the Licensed Clinician and you will work in a hybrid model with community-based travel required.
Responsibilities:

Per Diem (As Needed) Shift
As a Respite Worker for the Permanency Resource Center, you will provide respite services to youth in adoptive, guardianship and kinship families. Respite may also be available to families as part of the service plan and can occur at parent support groups or other PRC events and will be designed to be consistent with the children's needs and interests. Outings or activities with individual youth will be coordinated with the supervisor and parent. Respite provided as part of a group activity at a support group will be designed to allow parents the opportunity to have time separate from their children while they attend a support group. You will report to the Case Manager Supervisor or designated supervisor at the event. You will work onsite with youth in the community.

- Provide supervision for youth while parents attend support group.
- Provide respite as assigned to a youth as part of their service plan which can include activities in the community or in the youth’s home if that is the agreed upon plan.

POSITION OVERVIEW
School Based Clinician
As a Clinician, you will work collaboratively with the school district and as part of an interdisciplinary team with identified students, by providing clinical counseling to the youth and their family in an outpatient mental health clinic, licensed by the New York State Office of Mental Health (OMH), imbedded within the School Building. As a Skillbuilder, you will provide targeted skill training to help with independent living and social skills. Goals that the Skill Builder will work on could be Career Planning, Self-Care, Daily Living, Housing, and Money Management, including establishing and maintaining interpersonal relationships, how to accept no as an answer, handling conflict with peers and authority figures, and using their coping skills when feeling dysregulated. You will report to the Program Coordinator and you will work onsite.
Responsibilities:
- At a minimum, 50% of the work week is spent face-to-face with clients to provide services designated in the Treatment Plan.
- Provide services based on the youth's needs and preferences as identified in the Treatment Plan.
- Work with treatment team members to identify goals and objectives of treatment, including youth, families, school, and counselors.
- Complete all required documentation and submit it to the Supervisor within the required timeframes.
- Ability to work with diverse consumers who have mental health challenges and may be developmentally delayed. Understand their independent living needs to work with the case manager to find resources in the community for educational and vocational skills and programming for discharge.

As a Peer Advocate, you will in the Permanency Resource Center to assist adoptive, guardianship and kinship/custodial families and case managers in helping families and youth access services. You will assist in the development, scheduling and provision of family engagement activities and support groups. You will report to the Permanency Resource Center Case Manager Supervisor and will work both onsite and remote with community-based travel required.

As a Children's Health Home Care Manager, you will engage youth and families through outreach activities until enrollment into the Children's Health Home program. You will coordinate and monitor subsequent services to meet the needs of your members. You will report to a program supervisor, working in a hybrid model. Work will involve a combination of remote, office, and community-based settings. Local travel and personal transportation is required.
Responsibilities:
- Level I - Provides care management services to children with low and medium acuities. Care Manager will provide the required one or two services each month based on acuity and need. One of these contacts is to be in-person with the member.
- Level II - Provides care management services to low, medium, and high acuity members. Care Manager will provide a minimum of two services each month, one of which is to be in person with the member.
- Engage children, youth, and families with outreach and enrollment activities for successful enrollment into a children's health home
- Gather and complete assessments and enter documentation that confirms eligibility and acuity, identifying both strengths and areas of need
- Work w/children, youth, families, and service providers to create a comprehensive Plan of Care
- Identify and engage additional providers (e.g. natural supports, parent advocates, primary care physicians) with youth and families for the execution and continuation of the Plan of Care
- Convene and conduct Interdisciplinary team meetings to review Plan of Care; identifying progress, continued needs, and confirming ongoing eligibility
- Document progress and arrange for transitions between systems, placements, and higher levels of care following changes in eligibility and acuity.
- Use NY State and Health Home information systems to input required Health Home documentation
- Comply with Medicaid and program billing regulations
- Complete all required and recommended trainings
Requirements and Education:
- Level 1 - Requires an Associate's degree in Social Work or a related field, with 2 to 5 years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
- Level 2 - Requires a Bachelor of Arts or Science degree, and two years of experience providing direct services to children, persons diagnosed with mental disabilities, developmental disabilities, or substance abuse.
